More than 90 per cent of Canadians have said they would prefer to die at home. The Community Program at The Dorothy Ley Hospice was created to provide care and support enabling individuals to remain in their own homes for as long as possible.
Our Community Program consists of:
Regulated health care professionals (nurses and social workers) visit with palliative individuals and their caregivers to discuss and explore their needs, help them navigate the health care system and coordinate and monitor their support services.
Often families need extra support to offer social
interaction for the palliative individual, respite for the caregiver,
transportation to and from doctors appointments, preparation of light
meals and snacks and help with groceries. We have trained volunteers
who are able to visit individuals in their homes or other settings, to
give caregivers a break, to listen and to offer comfort.
